Most miscarriages start with bleeding and pain of some sort, whether mild or severe. The pain is usually in the lower back, stomach or thighs. Spotting may be of any color from bright red, as in fresh blood, to dark brown, as with old stale blood, and is sometimes mistaken for a menstrual cycle.
